Interventional Pain Management Physician and Co-Founder of Gold Coast Pain and Wellness
Dr. Shahriar Pirouz is board-certified in Pain Medicine, Anesthesiology, and Internal Medicine, and a co-founder of Gold Coast Pain and Wellness Center. His practice is devoted to pain medicine, but the extensive training in Internal Medicine and Anesthesiology shapes how he approaches it. Many patients living with chronic pain are also managing complex medical problems— factors that change which procedures are safe, which medications are appropriate, and how risk should be weighed. He evaluates those questions directly and communicates clearly with the physicians managing the rest of a patient’s care.
He completed his training at UCLA and Cedars-Sinai Medical Center and has practiced interventional pain management on California’s Central Coast for more than ten years. His clinical work includes image-guided epidural and joint injections, medial branch blocks and radiofrequency ablation, sympathetic and peripheral nerve blocks, and spinal cord stimulation — paired with conservative care, careful medication management, and coordination with physical therapy, behavioral health, and surgical colleagues when appropriate.
A first visit is a diagnostic visit. Dr. Pirouz reviews prior imaging and records, performs a focused examination, and explains what he believes is generating the pain and why. Some patients leave with a procedure scheduled. Others leave with a medication change, a referral, or a recommendation to wait. Patients are told which one applies to them, and why.
Dr. Pirouz holds hospital privileges at Arroyo Grande Community Hospital, Marian Regional Medical Center, and Sierra Vista Regional Medical Center, and surgical facility privileges at Coastal Surgical Institute and Galileo Surgery Center. He serves as a Qualified Medical Evaluator under California workers’ compensation, provides specialty eConsult services to primary care physicians across the state, and has performed expert medical review for the Medical Board of California.
He co-founded Gold Coast Pain and Wellness Center to bring accessible, high-quality interventional pain care to San Luis Obispo and northern Santa Barbara counties, with offices in Pismo Beach and Templeton. He lives on the Central Coast with his wife and two daughters, and considers it a privilege to care for the same community his family calls home.
